Note - All Grand Officers and Pages should be registered with their assemblies.  They will not have a separate registration form.

Registration and checks must be received by August 7th.  If payment is made via PayPal, the form can be emailed without a need to mail.  I will confirm each registration received so if you do not receive a confirmation, please inquire.
